Portal Form Request and How to Send a Message

HOW TO GET A FORM THROUGH “MY KIDS CHART” (PATIENT PORTAL):
The process for requesting and receiving forms has changed with our new portal (“My Kids Chart“).

In order to request a form, you must be signed up for My Kids Chart (fill out the registration form) and be current with the yearly Administrative Fee.

Once you have access to your child’s portal account send a message to us via the portal requesting the form you want. If it is a school form, we will generate the form and there is no need to give to us first. If it is a different form, either send the message through the portal and an electronic version to forms@tcfap.com or bring to the office.

Once the form is completed you will be sent a message through the portal with a link to the form.
